Port Insert Test Sheet Requirement:

Per Kirby Morgan Dive System’s Bulletin #4 of 2025. June 10, 2025:

Testing of the Port Inserts should be conducted ONCE A YEAR, and/or whenever damage to the Port Insert is present or suspected, using KMDSI P/N 525-115, Thread Insert Testing Block Kit for all fiberglass Kirby Morgan® helmets and both models of BandMasks®.

Annual Inspection/Overhaul/Maintenance Checklist MUST include a signed and completed Port Insert Test Sheet. Annual Inspection/Overhaul/Maintenance Checklists recorded without the Port Insert Test Sheet may be considered incomplete.
